Effective April 19, 2021, those NEWLY REGISTERING to receive a vaccination in New Hampshire must create their own individual account and schedule their own appointment, including spouses and household members. One appointment equals one vaccination. Appointments for individuals ALREADY REGISTERED as a plus-one prior to April 19, 2021 will be honored as scheduled. No additional action is needed for those registered as a plus-one prior to April 19, 2021.

 

Due to FDA emergency use authorization requirements, recipients who are 12 to 17 years old may only receive the Pfizer vaccine, which is available at the following locations:

  1. All state-managed fixed sites and Walgreens locations
  2. Androscoggin Valley Hospital (Berlin)
  3. Upper Connecticut Valley Hospital (Colebrook)
  4. Weeks Medical Center (Lancaster/Whitefield) 
  5. Littleton Regional Healthcare (Littleton)
  6. Huggins Hospital (Wolfeboro)
  7. Memorial Hospital (North Conway)
  8. Concord Hospital - Laconia (Laconia)
  9. Dartmouth Hitchcock Medical Center (Lebanon)

Please note, anyone under 18 must have consent from a parent or guardian. This can be verbal consent from the parent or guardian onsite at the time of the vaccination, written consent that can be filled out by the parent or guardian in advance or while onsite, or an electronic consent form that can be signed by the parent or guardian at the time of registration.

You will need to register and create an account in New Hampshire’s Vaccine & Immunization Network Interface (VINI) before you can schedule your appointment(s). Once registered you will receive an email to activate your account and schedule your appointment(s). You can also call 2-1-1 for help with registering and scheduling.

Due to FDA authorization, the COVID-19 vaccines are not currently available to individuals under the age of 12. Continue to check nh.gov/covid19 for updates and call 2-1-1 with questions. COVID-19 specific call-takers are available Monday through Friday 8:00AM – 8:00PM.
