How Does the Registration Process Work?

You must complete the steps below to register in order to receive a vaccination appointment.

  • Immunocompromised state (weakened immune system) from solid organ transplant
  • Obesity
  • Severe Obesity
  • Pregnancy
  • Sickle cell disease
  • Other high risk pulmonary disease
  • Type 2 Diabetes Mellitus

Ambulatory care health workers and first responders at most risk must register. This form must be completed by the individual being vaccinated. The individual will receive information about the vaccine, which must be reviewed prior to arrival at the vaccination clinic.

Long-term care residents are receiving their vaccinations from the long-term care facility.
